The Asst.P.F. Commissioner (AG-31) through letter 1st cited, requested Accounts Officer (FPS)/ APSRTC EPF Trust, to ensure while submitting the claim in Form No.10 D (for members pension), Form No.10 D (For Widow/Widower/Children Pension) and Form No.10 C (for Scheme Certificate/Withdrawal Benefit) that the enclosures as detailed in enclosed Annexure-I, Annexure-II & Annexure-III respectively are appended to the claims. 

 

Accordingly, the AO(FPS) through letter 2nd cited, communicated the said letter together with its enclosures to all the Unit Officers for necessary compliance.  A copy of the letter 2nd cited, together with its enclosures is annexed to this circular  for ready reference of the Unit Officers.

 

Instructions were issued through circular 3rd cited, advising all the Depot Managers to obtain the details of Drivers employed by the Hire bus owners as specified therein and furnish the same to Secretary, APSRTC EPF Trust along with the first remittance of PF&EDLIF contributions.

 

Instructions were also issued through Circular 4th cited, on maintenance of certain records to meet the statutory requirements, duly specifying the same in respect of contract labour engaged by the contractors and forwarding their claims in the event of cessation or death as the case may be.

 

But, instances have come to notice that some of the Unit Officers are submitting claims for sanction of EPS with insufficient information/documents resulting in return of the same by the Secretary, PF-Trust for furnishing full information and there by delay in sanction of EPS.  Further, the Unit Officers are not in a position to submit the settlement claims in respect of the Contract Labour / Hire Bus Drivers, especially in cases of death, due to non availability of the required statutory data that has to be maintained in the Units invariably.  Non-settling of the claims in time will entail much hardship to the employees/family members of the deceased and  additional financial burden to the Corporation besides penal action by the RPF Authorities.

 

            In order to avoid legal complications and financial hardship to the employees/Ex-contract workers/Hire Bus drivers and the families of the deceased, all the Depot Managers/Unit Officers are once again advised to maintain the data/records that are required to be maintained under EPF&MP Act, 1952 as communicated through references cited, and forward the filled in settlement Claim Forms of EPF, EPS and EDLIF in all respects to the Secretary, APSRTC EPF Trust.

 

            Failure to comply with the above instructions and to maintain the data/records required under EPF & MP Act, 1952 will be viewed severely and the Depot Manager/Unit Officer concerned will be personally held responsible for all consequences that may arise.

Pension claim preferred by the Scheme Certificate Holder

To enclose the Original Scheme Certificate where already it was issued and pension is claimed consequently along with the Form No.10D, 3 Joint Photos, SB Account No. first page of Bank Pass Book duly attested the employer may be forwarded.

Medical Certificate from the Board

If the member is disabled and claiming for disabled pension.

Date of Birth of Children

Proof of date of birth of children below 25 years.

If the claim is preferred by the nominee of Pensioner

If the pensioner or Scheme Certificate holder is died.  The nominee has to submit the copy of PPO or Original Scheme Certificate as applicable along with descriptive rolls, Group Photos, original death certificate, Proof of birth of eligible children, SB Accounts Number etc.
